Normally, your insurance carrier will send an explanation of benefits explaining what they have paid and what the patient’s portion is to pay. The explanation of benefits is a useful document to the patient for two specific reasons. First, it will help you to reconcile the payment amounts to those amounts reflected on your statements. Second, it will allow the patient to contact their carriers, if needed, for any questions regarding the amounts paid on those particular charges.
COLLECTIONS
If no further insurance is available or there has been an insufficient payment, the business office will make every effort to work with you to arrange for payment. If these efforts are unsuccessful, your account will be forwarded to an agency for collection. It is important to pay your bill on time to avoid the collection agency procedures. The collection agency will report your account to the credit bureau.

INSURANCE
At the time of registration, you will be asked for your insurance information. The process will rely on you to make sure that the information you give will be up to date and accurate in order to file your insurance claim in a timely manner. If you have multiple insurances, it is important that we know both your primary carrier and secondary carrier to ensure proper filings. We also need your legal name, current address and other appropriate information to properly identify you with our billing system, as well as with, your insurance carrier.
It is a priority for Schwartz Chiropractic to properly process your insurance claim, so we ask that if any insurance or demographic information changes, you notify us as soon as possible. During the registration process, you will be asked to fill out your confidential case history.
This form allows Schwartz Chiropractic to treat you as a patient, to file with your insurance carrier and have your carrier make payments directly to Schwartz Chiropractic. Please read the patient registration carefully. The office staff will be happy to answer any questions regarding its content. In addition to this form, Medicare recipients will be asked to sign an Advanced Beneficiary Notice (ABN).
FINANCIAL PAYMENTS
Schwartz Chiropractic requests the patient portion of charges be paid at time of service. However, if this is not possible, Schwartz Chiropractic will perform services that are indicated to be medically necessary. The patient’s insurance carrier will be billed immediately. In the event your insurance carrier is not prompt in making payment, Schwartz Chiropractic will need your assistance in contacting your carrier for payment. The amount you pay varies depending on your insurance coverage.
Most policies require patients to pay a deductible and a percentage of the remaining cost of services (co-payments). Schwartz Chiropractic will make every attempt to verify your insurance benefits prior to service. If at all possible, we will inform you of the approximate amount you will owe and arrange for payment.
FINANCIAL ARRANGEMENTS
If you do not have insurance or will have difficulty in paying your portion of the bill, financial arrangements may be approved, but it is important to remain timely with your payment to maintain your credit.
BILLING STATEMENTS
Schwartz Chiropractic will send a summary statement itemizing charges incurred by the patient after we have heard from insurance and/or if you have a balance. This statement will be mailed to the person responsible for payment, the guarantor. The amount billed to the insurance company will be reflected on the statement, if we have heard from them. Payment is expected within 30 days of receiving a bill from our clinic.
Spinal Adjustment/Acupuncture/Laser Cash Plan
This cash plan has been designed for our patients who prefer to pay cash or do not have health insurance. In order to receive the discount, payment must be paid in full ‘at the time of service’ on your first visit.
Based on Doctor’s recommendation, one of the following therapies will also be included with each appointment:
- Massage chair
- QGM - pain control therapy
- Foot massager
- Treadmill

Acupuncture needles ($5 per visit) are not included in the cash plan. When mens-o-matic graphing is necessary, it is an additional $5.00. If more than one therapy is recommended with your spinal adjustment or acupuncture appointment or laser therapy, there will be an additional $5.00 per therapy.
